Modify an active appointment for an employee due to employment changes in their role at the University. You can update HR system data for:
- Compensation (including increases to base pay and requesting additional pay)
- Position and job information (including job code, department, supervisor, standard hours, etc.)
- Position changes that involve the employee changing departments within a college can be submitted using this form.
- Position changes that involve the employee changing colleges or administrative units would require a Hire an Employee TDX form.
- Status changes (including work breaks, sabbaticals, and professional development leaves of absence)
- Expected end/return dates
- Phased retirements
- How the employee is paid (hourly, salaried, 9- or 10-over-12-month appointments, etc.)
- Reclassification for Faculty and Administrative promotions
- (Note: Use the Reclassification TDX form for Labor-represented, Civil Service, P&A employees in a job family)
Multiple updates to an employee’s job record can be made on a Modify Appointment TDX form for an employee’s job record, provided the effective date applies to all changes requested. A separate form will be required if updating multiple job records for the employee or if the request involves more than one effective date.
To get started
- Ensure all necessary documents and approvals have been obtained.
- Send supporting documentation to your local HR representative for the employee’s personnel file.
- For overpayments, inform the employee and stay in contact until the overpayment is collected. Refer to the Overpayment Collection process for details about next steps.
- Identify if there are any changes to the ChartField String related to your request, including Additional Pay requests. If you answer “Yes” to questions on the form related to ChartField Strings, your request will route to FinOps Payroll Accounting.
- Avoid entering private data into your TDX request, including comments or attachments.
- When ready to modify an appointment, click the Request Service button on the right sidebar to start a request with HR.
I have a Transfer, should I use the Modify form or Hire an Employee form?
Use the Hire an Employee form for all jobs posted. Use the Modify Appointment if you are requesting an existing Graduate Assistant to move from one position to another (e.g. from TA to RA).
